Your annual retainer fee includes all of the following:
Comprehensive Annual Physical Exam.
All routine laboratory work associated with the Annual Physical.
Same or next day appointments when you’re sick.
Our experienced and professional staff answers the telephone personally
when you call – there are no answering machines or voicemail.
Access to your personal doctor 24 hours a day, 365 days a year.
Telephone, fax, or e-mail questions or requests, as you prefer.
Unhurried appointments, none less than 30 minutes.
Prescriptions may be phoned,faxed, or sent electronically to your pharmacy to reduce waiting time.
Major recommended immunizations are provided at no additional charge.
No additional bills or paper work will be sent to you, or to your insurance company.
To permit outstanding service and access, the practice is limited to approximately 400 patients; after that, names will be placed on a waiting list.
Please note: The comprehensive exam includes a complete history & physical examination with Dr. Fields. It also includes an electrocardiogram (EKG) with interpretation, if medically indicated.
Laboratory work includes a CBC (Complete Blood Count), Chemistry panel, full cholesterol panel, thyroid screen and/or blood test for prostate cancer once yearly. If additional tests are desired, or recommended, a requisition will be provided to process through your medical insurance.
Covered immunizations will include influenza vaccine (annually), tetanus-diphtheria update (every 7-10 years), pneumonia vaccine every 10 years for individuals over age 55, or those at high risk for pneumonia. Also included is the meningitis vaccine for entering college freshmen. Other immunizations are not included, but may be available at an extra cost. As an added benefit, we are currently offering the shingles and HPV vaccines to eligible patients at manufacturer's cost, which represents a substantial savings.
